Several factors affecting sick building syndrome complaints in construction office workers in Jakarta

Introduction: Sick building syndrome is a condition in which people who live or work inside a building experience sudden health and comfort impacts related to time spent inside the building, in the absence of a specific disease or cause that can be identified. This study aims to determine the picture of the risk of sick building syndrome in office workers and complaints of sick building syndrome. Methods: Data obtained from several respondents through axial sampling. Data collection was carried out by filling out questionnaires related to complaints of sick building syndrome that had been validated and observed. Results: Factors that influence complaints of sick building syndrome in office workers of hospital construction projects in Jakarta are room temperature, room humidity, habits of leaving the room during rest, and odor in the room. And complaints that often arise, including complaints of headaches, fatigue as many as 39 people, sore eyes, and dry cough and dizziness. Conclusion: Work environment factors that include room temperature, room humidity, and odors in the room as well as behavioral factors that include the habit of leaving the room are driving factors for the occurrence of sick building syndrome in office workers of hospital construction projects in Jakarta.

Toward Restorative Hospital Environment: Nature and Art in Finnish Hospitals.

This study explores the integration of nature and art in recent hospital construction projects in Finland, focusing on public hospitals. Interviews with 15 stakeholders, including hospital executives, workers, designers, artists, and architects, reveal the value placed on nature and nature-themed art in hospital settings. The research question presented was: How nature and art are incorporated in Finnish hospitals in order to achieve a restorative hospital environment? Findings highlight themes that appeared in different hospitals: (1) the desired atmosphere, (2) nature and multisensory experiences, (3) social support, and (4) sense of connection and belonging. Bringing nature inside the hospital, whether through natural elements or artworks, emerges as a promising approach. Despite positive outcomes, challenges such as cost and maintenance persist, indicating the need for further research to optimize these initiatives. Overall, incorporating nature and art in hospitals has the potential to enhance healing and well-being for patients, families, and healthcare workers.

Formation and maintenance of organizational culture in collaborative hospital construction projects

Literature defines the significance of organizational culture to the performance of projects, emphasized in collaborative contracts increasing the importance of relational management issues. We have studied the challenges emerging in projects and described preconditions for managing the formation and maintenance of organizational culture to increase the project performance. We applied qualitative research approach focusing on two hospital construction projects utilizing relational contract model.Shortening timeline, dynamism in emerging and departing project participants and the background organizational cultures of the participants set challenges for the management of the organizational culture. We propose specific managerial actions to separate formation and maintenance phases of the organizational culture in projects. We have specified managerial preconditions for cultural level of underlying assumptions like psychological safety, trust, and collaboration and getting rid of old habits and attitudes, focusing to form mindset of collaboration and best for the project in formation and maintenance phases.

Long-term risks of respiratory diseases in patients infected with SARS-CoV-2: a longitudinal, population-based cohort study

In the post-pandemic era, growing apprehension exists regarding the potential sequelae of COVID-19. However, the risks of respiratory diseases following SARS-CoV-2 infection have not been comprehensively understood. This study aimed to investigate whether COVID-19 increases the long-term risk of respiratory illness in patients with COVID-19. In this longitudinal, population-based cohort study, we built three distinct cohorts age 37-73 years using the UK Biobank database; a COVID-19 group diagnosed in medical records between January 30th, 2020 and October 30th, 2022, and two control groups, a contemporary control group and a historical control group, with cutoff dates of October 30th, 2022 and October 30th, 2019, respectively. The follow-up period of all three groups was 2.7 years (the median (IQR) follow-up time was 0.8 years). Respiratory outcomes diagnosed in medical records included common chronic pulmonary diseases (asthma, bronchiectasis, chronic obstructive pulmonary disease (COPD), interstitial lung disease (ILD), pulmonary vascular disease (PVD), and lung cancer. For the data analysis, we calculated hazard ratios (HRs) along with their 95% CIs using Cox regression models, following the application of inverse probability weights (IPTW). A total of 3 cohorts were included in this study; 112,311 individuals in the COVID-19 group with a mean age (±SDs) of 56.2 (8.1) years, 359,671 in the contemporary control group, and 370,979 in the historical control group. Compared with the contemporary control group, those infected with SARS-CoV-2 exhibited elevated risks for developing respiratory diseases. This includes asthma, with a HR of 1.49 and a 95% CI 1.28-1.74; bronchiectasis (1.30; 1.06-1.61); COPD (1.59; 1.41-1.81); ILD (1.81; 1.38-2.21); PVD (1.59; 1.39-1.82); and lung cancer (1.39; 1.13-1.71). With the severity of the acute phase of COVID-19, the risk of pre-described respiratory outcomes increases progressively. Besides, during the 24-months follow-up, we observed an increasing trend in the risks of asthma and bronchiectasis over time. Additionally, the HR of lung cancer for 0-6 month follow-up was 3.07 (CI 1.73-5.44), and the association of lung cancer with COVID-19 disease disappeared at 6-12 month follow-up (1.06; 0.43-2.64) and at 12-24 months (1.02; 0.45-2.34). Compared to those with one SARS-CoV-2 infection, reinfected patients were at a higher risk of asthma (3.0; 1.32-6.84), COPD (3.07; 1.42-6.65), ILD (3.61; 1.11-11.8), and lung cancer (3.20; 1.59-6.45). Similar findings were noted when comparing with a historical cohort serving as a control group, including asthma (1.31; 1.13-1.52); bronchiectasis (1.53; 1.23-1.89); COPD (1.41; 1.24-1.59); ILD (2.53; 2.05-3.13); PVD (2.30; 1.98-2.66); and lung cancer (2.23; 1.78-2.79). Our research suggests that patients with COVID-19 may have an increased risk of developing respiratory diseases, and the risk increases with the severity of infection and reinfection. Even during the 24-month follow-up, the risk of asthma and bronchiectasis continued to increase. Hence, implementing appropriate follow-up strategies for these individuals is crucial to monitor and manage potential long-term respiratory health issues. Additionally, the increased risk in lung cancer in the COVID-19 individuals was probably due to the diagnostic tests conducted and incidental diagnoses. Factors influencing cost overruns in publicly funded hospital projects

In the context of Vietnam's strong economic development and increasing demand for healthcare, enhancing healthcare infrastructure and addressing the issue of overcrowding in public hospitals has become a crucial government objective. This study focuses on identifying the factors influencing cost escalation in public hospital construction projects funded by public investment. The research applies a non-probability sampling method, using convenient sampling and Referral/Snowball sampling techniques. A total of 120 individuals participated in the study, including investors, project managers, design and construction contractors, subcontractors, and relevant suppliers. The study's findings have identified and ranked the ten most significant factors affecting cost escalation in public hospital projects. These factors include inadequate functional design, inadequate functional design, delay in disbursing public investment capital, unforeseeable events, design errors leading to repetitive construction, impact of exchange rate fluctuations on imported medical equipment, unfavorable weather conditions during construction, delay in providing adjusted contract prices or approving adjusted investment amounts, complex geological conditions, and use of estimated quantity estimates in bidding when design is incomplete. The results of this research contribute significantly to the management of construction costs in public hospitals, particularly in the context of public investment funding.

The client's essential stakeholder collaboration activities at the front-end phase of a hospital construction project

PurposeThe research problem in this study is how a client (as a project owner) should organise early stakeholder involvement and integration in the front-end phase of a project. This study aims to create normative managerial statements as propositions from the client's perspective and to combine them into a set of activities enabling efficient organisation in the front-end phase of a hospital construction project.Design/methodology/approachAction design research (ADR) was carried out in a large hospital construction project where the first author acted as an “involved researcher” and the other authors acted as “outside researchers”.FindingsThe authors created seven normative managerial propositions that were verified by the case project stakeholders and developed a managerial framework describing the client's essential stakeholder involvement and integration activities in the front-end phase of a hospital construction project based on these propositions. The authors have also depicted the subphases of the front-end phase: value definition phase in the client permanent organisation, value proposition phase in the client Programme Management Office (PMO) and finally development phase in the alliance organisation ending on the final investment decision.Practical implicationsThe collaborative contract delivery model enables the early involvement and integration of stakeholders. It has been somewhat surprising to note the extent to which collaborative contracts change the client role in the project front-end. The results offer practical activities for how clients can manage front-end activities in collaborative contracts.Originality/valueThe case project offered a platform to analyse how the collaborative contract delivery model changes the emphasis of activities in the front-end of a project. One of the key benefits of collaborative contracts is that development, design and delivery occur partially in parallel, thereby enabling contributions from production to be included in the design and development. The benefit of having a real-life case under study provides the possibility to triangulate and analyse rich data, however limited by the qualitative case method.

Dissecting shared genetic architecture between obesity and multiple sclerosis.

Observational studies have associated obesity with an increased risk of multiple sclerosis (MS). However, the role of genetic factors in their comorbidity remains largely unknown. Our study aimed to investigate the shared genetic architecture underlying obesity and MS. By leveraging data from genome-wide association studies, we investigated the genetic correlation of body mass index (BMI) and MS by linkage disequilibrium score regression and genetic covariance analyser. The casualty was identified by bidirectional Mendelian randomisation. Linkage disequilibrium score regression in specifically expressed genes and multimarker analysis of GenoMic annotation was utilised to explore single-nucleotide polymorphism (SNP) enrichment at the tissue and cell-type levels. Shared risk SNPs were derived using cross-trait meta-analyses and Heritability Estimation from Summary Statistics. We explored the potential functional genes using summary-data-based Mendelian randomization (SMR). The expression profiles of the risk gene in tissues were further examined. We found a significantly positive genetic correlation between BMI and MS, and the causal association of BMI with MS was supported (β=0.22, P=8.03E-05). Cross-trait analysis yielded 39 shared risk SNPs, and the risk gene GGNBP2 was consistently identified in SMR. We observed tissue-specific level SNP heritability enrichment for BMI mainly in brain tissues for MS in immune-related tissues, and cell-type-specific level SNP heritability enrichment in 12 different immune cell types in brain, spleen, lung, and whole blood. The expressions of GGNBP2 were significantly altered in the tissues of patients with obesity or MS compared to those of control subjects. Our study indicates the genetic correlation and shared risk genes between obesity and MS. These findings provide insights into the potential mechanisms behind their comorbidity and the future development of therapeutics. Development levels of stakeholder relationships in collaborative projects: challenges and preconditions

PurposeThis article aims to address the confusion related to the meanings of interorganisational cooperation, control, coordination and collaboration in collaborative projects by developing a conceptual framework. From this, the authors aim to describe the links among these concepts in terms of development levels of stakeholder relationships. In addition, the authors aim to identify challenges and preconditions in relation to developing relationships at different levels.Design/methodology/approachThe authors have adopted the directed approach of qualitative content analysis method to validate and extend the conceptual framework of this study. The context of this study is a large hospital construction project located in northern Finland.FindingsThe findings of this study suggest that collaboration is a multilevel process of active engagement of multiple stakeholders. These stakeholders must have a high degree of shared understanding in terms of cooperation, control and coordination to achieve the mutually desired outcomes. This study also identifies the challenges that project stakeholders could face in developing collaborative relationships and propose preconditions for the same.Practical implicationsThis study provides a better understanding for project managers to manage interorganisational collaborative construction projects successfully. The outcome of this research would be beneficial to project management team to deliver dispute-free construction projects.Originality/valueExisting practical research on the development of relationships at different levels in collaborative construction projects is limited. This study offers a framework for the same which is validated in a real-life project.

High risk and impact factors on construction management process − case study of COVID-19 of a hospital in Iraq

The pandemic of COVID-19 crisis suddenly appeared worldwide, and there were no proper crisis management procedures taken in advance to contain such a problem. Moreover, it has had harmful and unmeasured consequences on most life sectors, including construction, which has been severely impacted during the pandemic. This paper aims to review factors with a high-risk impact on hospitals' construction projects during the outbreak of COVID-19 in developing countries by considering the case of Iraq's construction sector. A mixed methodology of qualitative and quantitative research approaches was used. The qualitative method involved (1) a literature review and (2) semi-structured interviews to identify high-risk factors that affect construction projects during the outbreak of COVID-19. The quantitative method involved (1) collecting survey data from 205 industry professionals and experts to determine each risk factor's importance and influence. Cronbach's alpha test to check the reliability of the collected data set, and (3) a fuzzy inference system method was used to assess the impact of each factor on construction projects during the pandemic. The findings of this study indicate that the construction industry has been hit severely during the COVID-19 in Iraq. Moreover, 17 high-risk sub-factors under six main construction factors involve health and safety, risk management, management deficiency, financial, supply chain management and contractual impact hospital construction projects. Furthermore, the most significant risk factors had impacted hospital construction projects by calculating their means were (1) commitment to safety and health recommendations with a level of impact equal to 4.81; (2) risk management procedures with a level of impact equal to 4.63; (3) equipment delivery delays with a level of impact equal to 4.54; (4) worker acceptance of COVID-19 vaccination with a level of impact equal to 4.36; (5) increase of price material with a level of impact equal to 4.18; (6) lack of use of the latest technology with a level of impact equal to 4.09; (7) delay in payments with a level of impact equal to 3.90; and (8) lack of training to deal with the pandemic with a level of impact equal to 3.81. As such, this paper contributes by providing effective policies and measures to mitigate the impact of COVID-19 on hospital construction projects in Iraq.

Identity formation and maintenance in a large hospital construction project

Earlier research has demonstrated that in a project organization, project identity is a central leadership vehicle that offers a useful means for examining and developing shared interests and goals among project participants. However, little research has focused on project identity formation and identity maintenance.We focused on the case of a large hospital construction project and used a qualitative research method to create propositions describing the essential identity formation activities that the program management office (PMO), the project director, and together with the project management team need to intentionally undertake in the early phases of a hospital construction project and corresponding identity maintenance during the project implementation period.The findings indicate that identity creation and formation is initiated from the first meetings at the project front end and naturally evolves, but significant intentional actions need to be organized and controlled at the front end. Identity maintenance as a deliberate managerial action should take place during project implementation to keep identity alive as a positive resource, not resulting in the identity fading away to something else.

Early prediction of treatment response to neoadjuvant chemotherapy based on longitudinal ultrasound images of HER2-positive breast cancer patients by Siamese multi-task network: A multicentre, retrospective cohort study.

SummaryBackgroundEarly prediction of treatment response to neoadjuvant chemotherapy (NACT) in patients with human epidermal growth factor receptor 2 (HER2)-positive breast cancer can facilitate timely adjustment of treatment regimens. We aimed to develop and validate a Siamese multi-task network (SMTN) for predicting pathological complete response (pCR) based on longitudinal ultrasound images at the early stage of NACT.MethodsIn this multicentre, retrospective cohort study, a total of 393 patients with biopsy-proven HER2-positive breast cancer were retrospectively enrolled from three hospitals in china between December 16, 2013 and March 05, 2021, and allocated into a training cohort and two external validation cohorts. Patients receiving full cycles of NACT and with surgical pathological results available were eligible for inclusion. The key exclusion criteria were missing ultrasound images and/or clinicopathological characteristics. The proposed SMTN consists of two subnetworks that could be joined at multiple layers, which allowed for the integration of multi-scale features and extraction of dynamic information from longitudinal ultrasound images before and after the first /second cycles of NACT. We constructed the clinical model as a baseline using multivariable logistic regression analysis. Then the performance of SMTN was evaluated and compared with the clinical model.FindingsThe training cohort, comprising 215 patients, were selected from Yunnan Cancer Hospital. The two independent external validation cohorts, comprising 95 and 83 patients, were selected from Guangdong Provincial People's Hospital, and Shanxi Cancer Hospital, respectively. The SMTN yielded an area under the receiver operating characteristic curve (AUC) values of 0.986 (95% CI: 0.977–0.995), 0.902 (95%CI: 0.856–0.948), and 0.957 (95%CI: 0.924–0.990) in the training cohort and two external validation cohorts, respectively, which were significantly higher than that those of the clinical model (AUC: 0.524–0.588, Pall < 0.05). The AUCs values of the SMTN within the anti-HER2 therapy subgroups were 0.833-0.972 in the two external validation cohorts. Percepatan Waktu Pelaksanaan Pembangunan Gedung Rumah Sakit Aisyiyah Kabupaten Ponorogo Menggunakan Metode PERT (Program Evaluation and Review Technique)

Project scheduling is one of the important factors for successful project implementation that can be achieved in accordance with the initial planning. Problems that can hinder project management are limited resources, inaccurate reports and costly and costly.  The delay in the completion of the project itself is a very undesirable condition, because it can harm both parties both in terms of time or cost.  The purpose of this study is to determine the probability of accelerating the implementation time from the period obtained by taking into account the critical path and calculation of cost crashing in the Aisyiyah Ponorogo Hospital construction project.   The research method used in this study was to analyze the time schedule in the RAB by comparing the project schedule using the PERT (Program Evaluation and Review Technique) method. The data used is the duration of the project time to obtain critical trajectory results, standards and variances. The results of the scheduling analysis with the PERT method obtained a critical path with B-C-D-E-L-M with an acceleration of the implementation time of project activities for 49 weeks on the critical path and a non-critical path for 79 weeks from the planned time.  buildings can speed up implementation time by a possible percentage of 92,073% with efficient calculation of crashing costs with the addition of working hours/overtime at a duration of 49 weeks rather than having to increase labor to achieve this probability.

A deep-learning system predicts glaucoma incidence and progression using retinal photographs

BackgroundDeep learning has been widely used for glaucoma diagnosis. However, there is no clinically validated algorithm for glaucoma incidence and progression prediction. This study aims to develop a clinically feasible deep-learning system for predicting and stratifying the risk of glaucoma onset and progression based on color fundus photographs (CFPs), with clinical validation of performance in external population cohorts.MethodsWe established data sets of CFPs and visual fields collected from longitudinal cohorts. The mean follow-up duration was 3 to 5 years across the data sets. Artificial intelligence (AI) models were developed to predict future glaucoma incidence and progression based on the CFPs of 17,497 eyes in 9346 patients. The area under the receiver operating characteristic (AUROC) curve, sensitivity, and specificity of the AI models were calculated with reference to the labels provided by experienced ophthalmologists. Incidence and progression of glaucoma were determined based on longitudinal CFP images or visual fields, respectively.ResultsThe AI model to predict glaucoma incidence achieved an AUROC of 0.90 (0.81–0.99) in the validation set and demonstrated good generalizability, with AUROCs of 0.89 (0.83–0.95) and 0.88 (0.79–0.97) in external test sets 1 and 2, respectively. The AI model to predict glaucoma progression achieved an AUROC of 0.91 (0.88–0.94) in the validation set, and also demonstrated outstanding predictive performance with AUROCs of 0.87 (0.81–0.92) and 0.88 (0.83–0.94) in external test sets 1 and 2, respectively.ConclusionOur study demonstrates the feasibility of deep-learning algorithms in the early detection and prediction of glaucoma progression.FUNDINGNational Natural Science Foundation of China (NSFC); the High-level Hospital Construction Project, Zhongshan Ophthalmic Center, Sun Yat-sen University; the Science and Technology Program of Guangzhou, China (2021), the Science and Technology Development Fund (FDCT) of Macau, and FDCT-NSFC.

Organising methods enabling integration for value creation in complex projects

PurposeThe purpose of this paper is to identify the areas and logic of integration of different stakeholders using different methods and to analyse their applicability and challenges in practical projects. The main aim is to describe how these different methods impact value creation.Design/methodology/approachAction design research was carried out in a large hospital construction project where the first author acted as an “involved researcher” and the second author acted as an “outside researcher”. Two workshops were organised to evaluate the direct and indirect challenges and benefits of the applied four methods and to explain how different methods enable value creation.FindingsAll the studied methods provide good results in terms of usability and commitment to the aims of the project, thus delivering the direct benefits expected. Process, people and tools logic works well in this case project when applying the methods properly. Significant evidence was provided on secondary deliverables of the methods, and all analysed methods had a significant impact in the area of leading people, clarifying what “focus on people” means and how it is enabled.Practical implicationsFocus on people can be achieved through different operative methods if applied in the right way. It is necessary to select the most suitable methods based on all the direct and indirect deliverables.Originality/valueThis case project offered a platform to analyse integration methods in a real-life project using the collaborative contract method. The authors were able to participate in the analysis by taking action from the very beginning of the project in terms of training, learning, continuous development and coaching of these methods and evaluating the applicability.

Modeling a construction project in a matrix-based framework for managing requirement changes

Changes are inevitable in a construction project and can impact project’s time, cost, other aspects such as labor productivity, safety, and working relationships. A key challenge of change management is to identify changes and predict their impacts before implementation so that impacts can be minimized. However, different set of entity types in a construction project such as teams, components, deliverables, activities and parameters, as well as complex interdependencies that exist among these entity types, makes difficult to manage changes. To predict changes and their impacts holistically, entity types should be modeled in an integrated manner explicitly. Therefore, objective of the study is to develop a matrix-based framework to predict changes by modeling multiple entity types in a construction project. The proposed matrix-based framework integrates two matrix-based modeling concepts: Axiomatic Design (AD) and Multiple Domain Matrix (MDM). AD principles are utilized for early change identification. MDM principles are used for tracking change propagations across multiple entity types. A schedule risk model is proposed for estimating delays due to changes in entity types. The scope of study is to predict delays due to requirement changes. Framework is validated using data from an ongoing hospital construction project. The findings revealed framework can identify and predict all possible changes accurately.

Every construction project, whether high-tech or simple, has risks in the implementation process. there-fore it is necessary to establish a Project Safety Management (PSM), to minimize the risk of occupational safety and health (OSH) of construction project. However there are still many construction companies that are lacking in identifying risks of occupational safety and health(OSH) of construction project and implementing an occupational Project Safety Management (PSM)) in the process of implementing a con-struction project. The purpose of this study was to determine the level of risk occupational safety and health based on existing risk factors, and to measure the application level of SMK3 in a construction pro-ject. In this study, the object of the case study was the construction project of the Mangusada Badung Regional General Hospital, at the time of finishing work. After carrying out risk identification, 4 risk fac-tors are obtained, namely human, material, equipment and environmental factors. then the results of the identification are processed, and the level of risk obtained from the implementa-tion of the Mangusada Badung Regional General Hospital Development Project is included in the Medium Category, with a risk value of 6. Meanwhile, the implementation of the Project Safety Management (PSM) of the Mangusada Badung Regional General Hospital Construction Project was included in the good cate-gory with a total application value of 83%. Hopefully this research will serve as a reference in identifying occupational safety and health (OSH) risks, and a reference in implementing the Occupational Project Sfety Manajemen (PSM).

Analysis of Important and Difficult Points in Feasibility Study of Comprehensive Hospital Construction Project

The objective is to evaluate 22 time and 12 cost forecasting methods based on Earned Value (EVM/ES) under three assumptions of future performance and in terms of the opportunity, accuracy, and stability of the forecasts. Little attention has been paid in the literature to future performance assumptions and evaluation under these parameters. Method: A total of 6,951 entry data points were used applying statistical measures of error and dispersion based on empirical evidence from 34 hospital construction projects in Mexico. Findings: Using contractor payment estimates as Actual Cost (AC), the more predictive time forecasting methods are those under the assumption that future performance will be as planned and, for cost, under the SPI(t) Schedule Performance Index. Relevance: The research shows the conditions under which the diverse methods offer timely and realistic predictions to be used as a reference in the monitoring and control of hospital construction projects. The limitation of the research consists of using durations and costs from completed projects without considering the possible changes formalized in contractual modifications throughout project’s execution. Future research using EVM/ES input data from the project owner's perspective is suggested.
